What's the difference between a private PPO plan and a marketplace plan?

Marketplace (ACA) plans are guaranteed-issue and can be heavily subsidized if your income qualifies, but they often pair a high premium with a high deductible and a narrow local network. Private PPO plans are medically underwritten, which means health history matters — but for a healthy applicant they frequently cost less and travel on a broad nationwide network. Neither one wins for everybody, which is exactly why a licensed advisor should look at your situation before you choose.

Can I really enroll any time of year?

For private plans, generally yes. The six-week open-enrollment window applies to marketplace coverage. Private options aren't tied to it, so most applications can start on any day of the year, with coverage commonly beginning on the first of a following month.

What if I have a pre-existing condition?

There are private plan structures that cover pre-existing conditions, and there are ones that don't. Underwriting looks at your history and either accepts, rates, excludes, or declines. An advisor will tell you plainly which side of that line you're likely to fall on before you apply — and if a guaranteed-issue marketplace plan is genuinely the better route for you, they'll say so.

How do I know my doctor is in network?

Nationwide PPO networks include most major hospital systems, but no network includes everyone. Bring your doctors' names to the call. Your advisor can check specific providers against the networks available in your state before anything is submitted.

What makes the premium go up or down?

Mostly your age band, how many people are on the plan, your state, the breadth of the network, your deductible preference, and — for underwritten plans — your health history. Tobacco use matters on most plans too. Two people the same age in different states can see very different numbers.

What if Medicaid or a subsidy is my better option?

Then we'll tell you. If your household income puts you in Medicaid range or in reach of a meaningful premium tax credit, that's usually the stronger deal, and an honest advisor points you there even though it means they don't write the policy.
